What is RehabCare?
The primary goal of Phelps County Regional Medical Center’s RehabCare Center is to help patients relearn the basic skills that will allow them to return to independent living and working environments. RehabCare is the leading contract manager of physical rehabilitation services and the fourth largest post-acute hospital operator in the United States. Their goal is to provide top-notch post-acute care that helps people regain their lives and reintegrate into their communities.
A disability can have far reaching effects on a person’s life. Rehab treats the whole patient – physically, emotionally, and socially – by bringing together a team of highly trained professionals to deal with every aspect of patients’ treatment and recovery. As a result of a physical impairment caused by an illness or injury, a person is called upon to develop new skills, re-learn previous skills and make a series of life adjustments. RehabCare uses a variety of goal-directed activities to help patients learn to do as much for themselves as possible. Activities include one-on-one therapy, group treatment, community re-entry and adaptive leisure activities, patient and family education, training and support.
To meet the unique needs of each patient, all treatment plans are tailored to fit the individual. After a comprehensive evaluation of the patient’s needs, appropriate goals are set, and a treatment program is implemented with the intention of returning the patient to the com¬munity as independent as possible. The treatment program is constantly evaluated and revised during regular case conferences.
RehabCare is highly accredited and has consistently receives high marks on surveys.
PCRMC satisfaction surveys show that 98 percent of RehabCare Center patients would recom¬mend the program to a friend. The center also has an 84 percent discharge-to-home rate, whereas the national benchmark for all rehab centers rests at just 72 percent. Nursing homes have less than a 40 percent discharge-to-home rate.
