Why do you need to copy my insurance card and a photo ID?
Phelps County Regional Medical Center Lab needs this information in order to bill your insurance company. We ask for the photo ID to protect you from identity theft.

Why do you need my insurance information every time I have my blood drawn? Don’t you already have this information on file?
Phelps County Regional Medical Center wants to ensure we have your current insurance information so you are not unnecessarily billed for services.

Why do you ask for my social security number?
Your social security number together with your personal information is used in the process of verifying your identity.

Will my insurance cover my lab tests?
If Phelps County Regional Medical Center Lab is a participating provider with your health plan, your out-of-pocket costs are usually limited to co-payments, co-insurances, and/or deductibles. If we are not participating with your health plan we will bill your insurance company as a courtesy to you, but any amounts unpaid by your plan are your responsibility. We recommend that you contact your health plan if you have questions regarding your benefit coverage or how your services will be covered.

We know that insurance rules are sometimes confusing and we make every effort to assist our patients and families. However, as a member of a health plan, you have a responsibility to understand how your plan works and what you must do to maximize your benefits and minimize your costs. We encourage you to become familiar with your health plan benefits before you need them. Please contact your employer benefits office and/or health plan representatives for assistance.

Whom should I call with billing questions?
You may receive separate bills for services received at the Phelps County Regional Medical Center Lab, one for professional fees (services provided by a pathologist) and one for hospital fees (laboratory testing). Each billing area has a customer service team to help you with questions or concerns you have about insurance billing or the amount you owe:

Where can I get my test results?
Laboratory results are available from the ordering physician’s office. Copies may be picked up or mailed to you if you fill out a release of information form available at the lab reception area.

Can you send my test results to more than one physician?
We will send your results to the ordering physician. We can send the results, at your request or your physician's request, to another physician(s) if you provide their complete name and address or fax number.

Can you send my laboratory testing to a Lab Card or LabOne participating laboratory?
The Lab Card and Lab Card Select Programs are part of Quest Diagnostics. However, if you have a Lab Card and/or insurance card with the LabOne logo you can have specimens collected at Phelps County Regional Medical Center Lab draw site and testing performed at the PCRMC Lab. Your standard benefit for testing will apply and the LabOne benefit is not applicable. You may be billed a coinsurance co-pay and/or deductible.

If you wish to have testing sent to Quest Diagnostics, you will need to have the specimen collected at a participating Quest Diagnostics site. You can contact a Lab Card client service representative at 800-646-7788 for current draw site locations. Provider collection and handling fees may apply, and are subject to health benefit plan provisions.

Can you send my laboratory testing to Quest Diagnostics?
Phelps County Regional Medical Center Lab is not a Quest draw site. You can contact Quest Diagnostics at 800-377-8448 for current draw site locations.
